The Manager role will support Evercare’s Advanced Illness program by providing a holistic approach to case management, including addressing physical, emotional, spiritual and psycho-social needs of members and their families. This position directly supervises up to 15 Care Managers who are responsible for assuring that members and families meeting the criteria for Advanced Illness have current, anticipatory and advanced care plans in place and that members’ goals of care are respected and honored. You will also be responsible for developing effective working relationships with our business’ senior leadership at the site level and will work effectively across sister segments both internally and across the Enterprise.

This is a telecommuting position open to candidates anywhere in the U.S.

Primary responsibilities include:

* Takes lead role in setting direction and participating in developing new programs * Sets team direction, resolves problems and provides guidance to members of their own team. * May oversee work activities of other supervisors. * Ensures team meets established performance metrics and performance guarantees * Adapts departmental plans and priorities to address business and operational challenges. * Influences or provides input to forecasting and planning activities. * Product, service or process decisions are most likely to impact multiple groups of employees and/or customers (internal or external)

* Undergraduate degree or equivalent experience required; BSN preferred * Minimum of 3 years management experience * Unrestricted RN license

Additional Assets Preferred:

* Ideal clinical experience includes palliative care, hospice, home health, or community case management * Successful candidates will have previous management/supervisory experience; experience managing remote employees is beneficial * Ideal candidates will be highly organized and possess excellent communication skills * Certified Case Manager Certification (CCM) * Demonstrated experience developing workflows or implementing process improvements to gain operational efficiencies * Demonstrated experience driving operational performance and achieving established metrics * Excellent communication skills with the ability to interrelate with individuals at various levels and in various functional areas throughout the organization * Develop effective business relationships across business and product teams
